  1. Approximate Weight of device
  2. Brand/Model
  3. Location to be sent back to
  4. What’s wrong with the device
  5. Pictures of the Device

Once you have this information, make a customer support account and submit a ticket with the information and our team will reach out to you as soon as possible.
